Application for Alaska Driver’s License, Permit or Identification Card

There are many forms that you can fill out if you go to your local DMV office in Alaska. One such form is an Application for Alaska Driver’s License, Permit, or Identification Card. This is also known as form 478. Since a driver’s license is required in the state of Alaska, you will almost certainly have to fill out this form at some point. You can use this form to apply for a driver’s permit and to apply for an ID card, as well.

Application for Title and Registration

If you own a vehicle, then you will know that there are quite a few forms you can fill out that are relevant to the vehicle’s title and registration. You might need a replacement title for your vehicle, especially if you lost the original title. If so, you can apply for the replacement title with the Application for Title and Registration. If you sell your vehicle, you can apply for a transfer of ownership and title using this form, too. You can also apply for a transfer of registration using this application.

Notice of Change of Address

It is quite common for people in any state to move from one address to another at some point. Of course, this applies to residents in the state of Alaska. You do have to notify the Alaska DMV if your address changes, however. You can do so by completing the Notice of Change of Address and submitting it to the Alaska DMV. Application for a Social Security Card

You need some proof of ID in order to do a number of different things at the Alaska DMV. Your Social Security card is one good proof of ID and it is accepted by all DMV offices. If you never got a Social Security card, you can apply for a Social Security card by filling out the Application for Social Security Card and submitting it to your local Social Security office.

However, if you never got a Social Security card and you are over the age of 12, you have to apply for a Social Security card in person. You can also use this form if you need a replacement Social Security Card. Additionally, you can use the Application for Social Security card to change information on your Social Security number record or to correct information on this record.

Notice of Transfer Pending

You also should notify the DMV if you are intending to sell a vehicle. In order to do so, you can go here and complete the necessary steps in order to notify the DMV that you are going to sell a vehicle. You can go to the above website and send a Notice of Transfer Pending to the Alaska DMV online. Otherwise, you can fill out the Notice of Transfer Pending at the bottom of the vehicle title.
