The Colorado Department of Motor Vehicles (DMV) provides a variety of services for state motorists. The following are some of the more commonly needed and requested forms to enable daily travels on public roads in Colorado and other important DMV services.

Universal Title Application

The Colorado Department of Motor Vehicles provides vehicle owners with an Application for Title and/or Registration that enables them to title and register newly acquired vehicles.

Affidavit of Surviving Spouse

When a property owner dies and leaves titles vehicles and other properties behind, an Affidavit of Surviving Spouse can help to ensure it stays in the family.

Renew/Replace DL- Out of State for Extended Period

Many Colorado residents find themselves doing work and living out of state for extended periods of time. When a motorist is out of state and needs to renew a driver’s license, he or she can do that online as long as it has not been more than a year since the driver’s license expired.

A Form DR2989 enables Colorado residents who are licensed to drive in the state to replace their lost or stolen driver’s licenses while out of state for an extended period of time.

Title and Register a Vehicle While Temporarily Located Out of State

Colorado residents can download and submit an Application for Title and/or Registration while out of state and obtain a Colorado title and registration.

Driver History Abstract Request

You can obtain a copy of your seven-year driving record in Colorado by completing and submitting a Form DR 2489 Driver Record Requestor Release and Affidavit. You must include any required documents and pay the required fee.

Wheelchair Symbol Plate/Placard Application

Individuals can obtain wheelchair symbol license plates and placards that enable them to part in handicapper parking spots without being towed or fined. A Form DR 2219 Persons with Disabilities Parking Privileges Application enables qualifying motorists to obtain the wheelchair symbol license plate and placard.

Wheelchair Symbol Plate/Placard Checklist

The Form DR 2219 plate and placard application includes a checklist that your physician, registered nurse or other qualifying medical professional can use to indicate your physical condition and qualification for the wheelchair symbol plate and parking placard.

Refund Application

If you sell a registered vehicle with more than six months remaining on the registration, you can obtain a partial refund by submitting a Registration Fee Refund Request Form with the Colorado DMV.

Physician Certification for Blind or Disabled Person, Non-Driver ID Application

While many people are blind or disabled and cannot drive, they still need to get around and run errands. The Form DR 2219 includes a location for a physician, eye doctor or other medical professional to provide evidence enabling the applicant to obtain a non-driver parking permit to use handicapper parking spots.

Vehicle Registration Application

The same Application for Title and/or Registration that enables you to title your vehicle also enables you to register it and obtain a license plate.
