Person with a Disability Parking Permit Application (Form PA 3)
The Person with a Disability Parking Permit Application needs to be certified by a Hawaii Licensed Physician or nurse who is licensed in the state. Since Hawaii is separated from the mainland USA, you will need to complete an Out-of-State Vehicle Permit application if you have brought your vehicle to the state but won’t register it because the trip is too short. You also need to complete a vehicle safety inspection prior to applying for this permit. There is a $5 permit fee, and you also need to submit the shipping receipt or bill of lading to document your vehicle’s date of arrival.
