When you want to complete any kind of transaction with your vehicle, you’ll have forms to fill out and take to the Office of Motor Vehicles. Some forms are available online, while other services require you to come to the OMV in person.

Many of the services that you may want to complete will be available on the OMV website through the digital application. You can find this website here and click through to complete any transactions that are available online. For some processes, such as getting your license for the first time, you will need to go to the OMV to complete the registration process.

There are many forms that you may want to use that the Louisiana Office of Motor Vehicles offers on its website. For certain actions, such as getting your license or registering your vehicle, you may need to go to the OMV directly instead of filling out paperwork by mail or online. If you have questions about how to take care of a certain transaction involving your vehicle, you can reach out to your specific OMV near you. You can search by parish or city on the OMV website, and each location has its own address and phone number available.

Application for Road Skills Test

If you would like to apply for your Road Skills Test to get your license, then you should fill out form 2271 available on the Office of Motor Vehicles website.

Medical/Vision Form

If you need to complete a medical or vision form to be able to drive, then you should complete form 2015.

Affidavit of Heirship

If you are the heir to an estate and need to have a vehicle transferred into your name, fill out form 1696, the Affidavit of Heirship.

Bill of Sale of a Movable Consignment of Motor Vehicle

When you want to sell your vehicle, you may need to use a bill of sale. If so, you can use the Bill of Sale of a Movable Consignment of Motor Vehicle to meet that requirement.

Odometer Statement

When you sell or buy a vehicle, the Office of Motor Vehicles may want to have the odometer reading. You can use the odometer statement form available on the OMV website.

Physician’s Certification of Mobility Impairment

When someone has a mobility impairment, they may need a physician’s certification and to provide that to the Office of Motor Vehicles. This form is form 1966 and available on the OMV website.

Vehicle Application

To complete a vehicle application in Louisiana, fill out form 1799.
