The Michigan Secretary of State (SOS) provides many services that require the correct forms and payments in order to obtain them. The following forms are among the more commonly requested.

Universal Title Application

When a Michigan resident buys a vehicle, he or she must title it in order to use it. An Application for Michigan Vehicle Title enables you to obtain a title. You must include your name and address, vehicle make, model and VIN number and the mileage. If a lienholder exists, the state will need to know that, too, to protect the lienholder’s interest in the property. You will have to pay a use tax as well as fees to obtain the title. You can mail in the title application or go to a Michigan SOS service center to obtain a new title.

Affidavit of Surviving Spouse

When the owner of a vehicle dies and leaves title vehicles behind, a surviving spouse can file an Heir to a Vehicle form. That will help to transfer ownership to the surviving spouse.

Renew/Replace DL – Out of State for an Extended Period

Whenever a Michigan resident temporarily is located out of state and needs to renew or replace his or her driver’s license, the state enables renewals online and replacements by filing an Out-of-State Resident Application for a duplicate driver’s license.

Title and Register a Vehicle While Temporarily Located Out of State

You can complete and mail-in a Michigan Title Application for any vehicle that you buy and what to title and register in Michigan. You can complete the registration process online.

Driver History Abstract Request

Individuals and organizations may obtain a Michigan driver history record for an individual by completing and mailing in the request form along with the fee.

Wheelchair Symbol Plate/Placard Application

Individuals with disabilities can complete a Disability Parking Placard Application and obtain a license plate and placard that enables legal parking in handicap parking spaces.

Wheelchair Symbol Plate/Placard Checklist

The application includes a plate/placard checklist that your medical doctor, physician’s assistant or registered nurse must complete to affirm your qualifying conditions.

Refund Application

If you sell a vehicle and more than six months remain on the registration, you can file a refund request form to obtain a refund on the balance of the registration fee paid.

Next-of-Kin Registry Application

The state of Michigan does not operate a next-of-kin registry. If a relative passes on and leaves a vehicle with no heirs, the next-of-kin can file a Heir to a Vehicle form that works the same as a next-of-kin registry.

Physician Certification for Blind or Disable Person, Non-Driver ID Application

You can submit a non-driver ID application with certification for blind or disabled person, non-driver ID. The ID enables motorists who are driving with the ID holder to use handicap parking spots so that the cardholder can get around more easily.

Vehicle Registration Application

Completing and submitting an Individual Form TR-11L will enable a motorist to obtain a vehicle registration as well as a title. You can do renewals online or at a remote kiosk as well as an SOS service center.
