Whenever you want to sell your vehicle, register your vehicle, get a copy of your title, or have other transactions with the Missouri Department of Revenue’s Motor Vehicle Bureau, you will need to make sure you have the right forms filled out. The following are some of the most common forms that you’ll need. Additional forms may be found on the Missouri Department of Revenue’s website here.

Application for Motor Vehicle License

If you want to renew your license and haven’t gotten a letter or notice, you can use Form 184, Application for Motor Vehicle License, to do so. Take this letter to a Missouri licensing office or mail it in.

Application for Replacement Plate(s) and Tab(s) (Form 1576)

The Application for Replacement Plate(s) and Tab(s) (Form 1576) form can be used if you need to place your tabs or enhanced tabs by mail. You should mark that you are requesting enhanced security tabs if you need those.

General Affidavit (Form 768)

General affidavit, or Form 768, is used if your vehicle needs to have a safety or emissions inspection and has been kept outside of the state of Missouri for more than two months (60 days).

Application for Missouri Title and License (Form DOR-108)

The Application for Missouri Title and License is used if you need to apply for a duplicate title. You can do this in person or by mail using this form.

Notice of Lien, Lien Release, or Authorization to Add/Remove Name from Title (Form DOR-4809)

Notice of Lien, Lien Release, or Authorization to Add/Remove Name from Title (Form DOR-4809) is a form used if you have a lien on your vehicle and need to get your title replaced. Your local Department of Revenue office should have this form, or you can mail in the digital copy.

Bill of Sale

Using the Bill of Sale, you can provide proof of purchase after you transfer ownership of your vehicle or take ownership of another person’s vehicle. A Bill of Sale form has to be notarized to be legally binding in Missouri.

Odometer Statement of Explanation

If you need to disclose the odometer reading on your vehicle or the odometer does not read correctly for some reason, you may need to submit the odometer statement of explanation.

Application for a Disabled Person Placard

If you would like to get a disabled person placard, you can do so by filling out the Application for a Disabled Person Placard form 2769.

Odometer Disclosure Statement

When you buy or sell a vehicle in Missouri, you should include an odometer disclosure statement. Use form 3019 and submit it with any other paperwork for the vehicle.
