Application for Driving Privileges or ID Card (DMV 002)
The Application for Driving Privileges or ID Card is DMV 002. This application is available in English or Spanish. This form cannot be mailed.
Confidential Physicians Report (DLD 7)
If you need a physician’s report before you can drive, then you should fill out form DLD 7. This form is for the medical assessment of a driver to determine if they will be able to get or retain a license.
Change of Address Notification by Mail (DMV 22)
If you move to Nevada or within the state, you do need to notify the Department of Motor Vehicles of the move. You can do this with the application DMV 22. You may also change your license online, which can do on the DMV’s website. This form may be mailed and is available in English, Tagalog, and Spanish.
Application for Vehicle Registration (VP 222)
Application for Vehicle Registration, form VP 222, is needed if you’ll be applying for your vehicle’s original registration. This registration does need to be signed by the owner of the vehicle.
If you are looking to get a duplicate registration, use the form VP 013 instead. This form comes with a $5 fee that must be paid to the DMV.
Vehicle Inspection Certificate (VP 018)
Vehicle Inspection Certification VP 018, is a verification that your vehicle has been inspected by an authorized agent. This needs to be completed by a representative of the DMV, a police/peace officer, or another person who is authorized to do so.
Application for Disabled Persons License Plates/Placards (SP 27)
Fill out the Application for Disabled Persons License Plates/Placards (SP 27) if you would like to get a disabled persons placard for your vehicle or change your license plates to reflect that a person with a disability uses the vehicle.
Duplicate License Plate Application (SP 14)
Use Duplicate License Plate Application SP 14 if you need a new license plate because yours has been damaged or is no longer legible. This is not intended to be used for stolen plates, and the plates will be in the same style as before.
Bill of Sale (VP 104)
If you’re selling a vehicle or want a bill of sale for a vehicle you are purchasing, use form VP 104.
Application for Duplicate Nevada Certificate of Title (VP 012)
Application VP 012 is the application for a duplicate certificate of title. This is only for use if the vehicle was previously titled in Nevada.
