How to Obtain Your West Virginia Driving Record

If you need to get your West Virginia driving record for work or other reasons, you can request a copy from the West Virginia DMV. The DMV offers your driving record online, in person or by mail.

Applying to Get the Driving Record

To get your driving record, you need to submit the Driving Record Request (DMV-101-PS1) online, by mail or in person. For a driving record for another person, you will also need to complete the Release Authorization (DMV-101-PS2).

Once you fill out these forms, you will need to go online or in person to the DMV. If you would like, you can also mail in the request to:

WV DMV, Driving Records Section
PO Box 17020
Charleston, WV 25317
For more help getting a driving record, you can also contact the DMV by reaching out by email to DMVDrivingRecordFax@WV.gov or calling (304) 926-3952.

Required Documentation

If you would like to get your own driving record, then you will submit the Driving Record Request form online, in person or by mail. It costs $7.50 if you have your name and driver’s license number. It’s $8.50 if you do not have your license number but do have your date of birth and Social Security Number.

You will also need to include a copy of your license if you have it alongside an address where you’d like the record to be mailed.

If you’re seeking out someone else’s records, you will need their license number or Social Security Number and date of birth. You will also need to include a Release Authorization with your request if the form is not for yourself.

Not everyone will qualify to obtain another person’s information. If you receive a denial, then you can submit a form called the Message Forwarding Service to a WV Driver’s License or ID Card Holder. This will let the DMV know you have completed a form and have it sent to the licensee. They can then choose to respond to your request for information.

Fees for Getting Your Driving Record and Payment Methods

If you are seeking a driving record with a driver’s license number, then it costs $7.50. If you do not have a license number but do have a date of birth and Social Security Number, then the cost is $8.50.

Types of Driving Records

Most states have at least two kinds of driving records, certified and non. West Virginia is a little bit different in that regard. The West Virginia DMV only offers one kind of driving record. You can choose to request your driving record online or in person. It’s also possible to ask for it by mail or through email or fax. Your driving record may be used by employers or insurance companies to verify your driving history.
